Home Health QA/QI (RN)
Schedule
* 8 hours shift

We are looking for a Quality Assurance Nurse (Full time, Korean speaking is a plus), who is committed and passionate about homecare nursing. QI/QA SN optimizes quality of care across all health center departments and sites by undertaking efforts to review clinical competency, ensure regulatory adherence, prevent adverse events, and promote infection prevention.
Responsibilities:
• Coordinates the patient’s and family’s care and services including those of the team, social worker, chaplain, home health aide, volunteer and physician
• Delivers necessary information to the Director of Nursing
• Monitors the re-certification / re-authorization process
• Participates in Interdisciplinary Team Meetings as indicated
• Ensures and documents appropriate use of medications, durable medical equipment and supplies
• Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Dietary, Speech and Pharmacy
• Performs on site evaluations of HHAs and volunteers
• Integrates QI results into the established plan of care
• Participates in QI assessment routinely
• Participates in routine chart reviews
• Ensure compliance with all Federal, State and local regulations and standards of practice
• Complies with Medicare, Medicaid and State license requirements
Qualifications:
• Valid, unrestricted CA Registered Nurse (RN)
• Experience in Home Health / Home Care, as either a Case Manager, Care Manager, Care Coordinator, performing Case Management / Care Managment is a plus
• ICD-10 CPT home health coding experience
• Proficient in all documentation & record maintenance to ensure accuracy, patient confidentiality & compliance
• Effective verbal and written communication skills.
• Speak Korean (preferred)
Requirements:
• Valid, unrestricted CA Registered Nurse (RN)
• Experience in Home Health / Home Care, as either a Case Manager, Care Manager, Care Coordinator, performing Case Management / Care Management is a plus
• Medical-related educational background
• Legally Authorized to work in the U.S.
