Qualifications and Requirements:
Education/Training/Certifications:
• A current State of California Class B Commercial Driver’s License (with P endorsement for multiple passengers)
• Safe and clean driving records within the past 3-years are required
• Ability to obtain a LA DOT city permit
• Current BLS certificate required
Experience:
• Minimum of 1 year transporting the frail and elderly or driver training specific to PACE.
Duties and Responsibilities:
• Responsible for PACE passengers during transport, providing care in a manner which ensures participant satisfaction.
• Follows assigned schedule, makes adjustments to meet unanticipated changes while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to the Transportation Coordinator or designee.
• Operates equipment safely and in compliance with vehicle licensing regulations.
• Maintains and completes daily required records, such as Pre and Post Trip Inspections, gasoline purchases, etc.
• Immediately reports to the Transportation Coordinator any vehicle or cell phone operating problems, malfunction, or incidents.
• Transports participants to and from medical/specialist appointments and activity outings as requested by the interdisciplinary team per established transportation request process.
• Transports meals, durable medical equipment, pharmacy, home care supplies, or other items deemed necessary.
• Provides physical assistance to each participant when needed as s/he boards and disembarks from the program’s vehicles. Provides door-through-door assistance.
• Inspects the vehicle at the end of each day and maintains the vehicle in a neat and clean condition.
• Ensures vehicle equipment is maintained and operating within manufacturer’s recommendations, replacing supplies as needed.
• Functions as a member of the interdisciplinary team when requested.
• Communicates with Transportation Coordinator or designee daily to be updated on participant conditions/changes, • plan of care changes, or interdisciplinary team requests.
• Follows policies and procedures and OSHA safety guidelines.
• Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families.
• Handles potentially infectious fluids (e.g., blood, urine, emesis) with appropriate biohazard precautions and • practices Standard Precautions.
• Participates in and supports Quality Improvement initiatives.
• Participates in continuing education classes and any required staff and training meetings.
• Maintains any required certifications or licenses.
• Performs other duties as required or requested.

Experience:
• Minimum of 1 year working with the frail or elderly population or training and tested competency provided by K-day PACE.
Duties and Responsibilities:
• Participate as a member of the interdisciplinary team on an as needed basis.
• Provides Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) to support participants such as: toileting, showers, bathing, dressing, grooming, feeding/assisted dining, meal service, laundry, transfers and ambulation, range of motion
• Completes safe transfers utilizing Durable Medical Equipment (DME) as required - lifts, gait belt and hoyers
• Cleaning the bathroom used by the participant, may include emptying and cleaning commode
• Create and participate in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Recreational Coordinator.
• Monitor and assist participants with meals.
• Throughout the day, assist in maintaining clean and orderly day rooms, kitchen and personal care areas.
• Accompany enrollee to medical and other appointments.
• Perform and or assist with laundry, including clothes for participants
• Assist transportation staff with the loading and unloading of participants from vehicles.
• Provide Escort assistance on transportation vehicles when requested.
• Note changes in participant’s physical or emotional health and communicate this information to the Day Center Supervisor or designee.
• Maintain participant’s medical record and fulfill charting and reporting requirements.
• Assist the transportation department with participants’ takes home supplies.
• Act only within the scope of his or her authority to practice.
• Participate in and support Quality Improvement initiatives.
• Follow all Policies and Procedures and OSHA safety guidelines.

• Associates or Bachelor’s degree in health-related fields preferred.
• Education or training in geriatrics preferred.
Experience:
• Minimum 2 years of experience in an outreach/marketing, intake or sales role in a health care setting.
• Minimum of 4 years’ experience if no degree.
Duties and Responsibilities:
• Receives and provides information to referrals to the PACE Program; refers individuals to other appropriate resources as indicated.
• Accepts referrals and inputs appropriate information into appropriate tracking methodology(s) and EHR timely.
• Maintains current, written assessment and intake records inclusive of potential participant’s expressed wishes and the referral services provided, as appropriate.
• Provides information on eligibility for the PACE program to both the participant and caregivers (if applicable).
• Assesses prospective PACE participants in homes, nursing homes, hospitals or other settings as part of the intake process.
• Communicates timelines and deadlines for assessments and prospective participant enrollment status within the marketing and enrollment department, with PACE program leadership and the IDT.
• Participates in Interdisciplinary Team meetings when potential enrollees are assessed for enrollment into the program for purposes of providing and clarifying information obtained during the enrollment process.
• Assists with the financial eligibility process for potential PACE participants by reviewing Medicaid eligibility and facilitating Medicaid applications with necessary supporting documentation for approval.
• Ensures necessary paperwork related to participant enrollment is received by State of California and Medical Records staff on or before the appointed cut-off date.
• Assists with community engagement and marketing activities as directed by Marketing and Enrollment Manager or other marketing staff.
• Collects data and prepares reports in accordance with the State PACE Administering agency and CMS requirements.
• Assists in evaluating the intake and assessment process, identifying problem areas accordingly and making changes as recommended.
• Contacts identified key referral sources regularly to discuss possible program participants.
• Maintains ongoing relationship with identified individuals and agencies to ensure a clear understanding of PACE as a vital community resource.

Duties and Responsibilities:
• Coordinate and execute outreach, PR and community engagement activities to ensure consistent achievement of • monthly referral, home visit and enrollment goals.
• Meets or exceeds census enrollment targets.
• Provides individual and group marketing presentations to potential referral sources.
• Identify opportunities to collaborate with community organizations and execute against those opportunities in a thoughtful manner that reflects our mission and value proposition.
• Establish and maintain best in class relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
• Develop and implement an annual marketing plan to achieve monthly enrollment goals with the Marketing Director following State and Federal guidelines.

• Bachelor's Degree from an ADA accredited institution with a major in nutrition. Master’s Degree preferred.
• Registered Dietitian
•Current ServSafe Food Protection Manager Certification and Instructor Status with Current Food Handlers Card
• Valid Colorado driver’s license and unlimited access to reliable transportation with automobile insurance
Experience:
• Minimum of 1 yearʼs experience working with a frail or elderly population.
• Three (3) years of experience working as a Registered Dietitian in geriatric or managed care settings.
Duties and Responsibilities:
• Participant initial, scheduled, and unscheduled reassessments.
• Participate in developing an individualized plan of care in collaboration with the Interdisciplinary Team (IDT) to meet the participant’s nutritional needs.
• Performs nutritional assessments when participant’s condition changes to determine participant’s on-going needs.
• Makes recommendations to the primary care provider for changes in diet orders when the client needs a different consistency/texture of food.
• Based on the nutritional needs identified in assessments, ensures that participants and family receive nutritional counseling. Ensures participants and/or family members are able to do return verbalization of education.
• Develops menus in collaboration with the food services vendor to meet one-third RD nutritional requirements, provide cost effective food options, and are nutritionally sound for a frail elderly population.
• Develops patient educational material.
• Supervises food service manager and dietary aides. Completes initial and annual competency for each, including safe food handling.
• Assist in the implementation of department competency plans for food service-related staff.
• Responsible for oversight of and training on infection control measures in the kitchen and food service areas to maintain food safety.
• Participates in the orientation of new staff and volunteers. This includes providing specific training for any new staff or volunteer who is going to assist with serving food.
• Trains staff, volunteers, and participants in nutrition related topics.
• Participates in daily IDT meetings
• Participates in and support Quality Initiatives
• Contribute to research and other department activities as assigned.
• Provides age specific competent care to older adults served including knowledge of the aging process.
• Knows and adheres to the philosophy and goals of K–Day PACE.
• Maintain professional level and remain current with new advances.

Education/Training/Certifications:
• Possess a minimum of a master’s degree in physical therapy from an accredited university except for applicants who graduated prior to 1/1/2003 and possess a minimum bachelor’s degree in physical therapy or a bachelor’s degree and certification in physical therapy.
• Licensed as Physical Therapist in California
• Current driver’s license and proof of auto insurance
Experience:
• Minimum of 1 year working as a Physical Therapist.

Duties and Responsibilities:
• Conduct an in-person comprehensive physical therapy assessment on all PACE participants upon enrollment. Coordinate with the interdisciplinary team to develop a comprehensive plan of care for each participant.
• Conduct an in person physical therapy reassessment on an annual basis and as needed
• Participate as a member of the interdisciplinary team, communicating participant changes, collaborating on care planning decisions and coordination of 24 hour care delivery. Actively participate in the Intake and Assessment meetings.
• Evaluate participants to determine and assure appropriate prescription, needs, goals, history, complications, and precautions. Follow up with participants during treatment, explain precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with the interdisciplinary team
• Assess the need for adaptive durable medical equipment, specialized shoes and prosthesis. Assess participant’s home for safety and mobility. Recommend home modifications and or equipment.
• Plan physical therapy treatment programs according to recognized standards and policies.
• Administer physical therapy treatment programs consistent in achieving identified goals and according to recognized standards and policies. Confer with an interdisciplinary team on participant care issues.
• Document evaluation, assessment, plan and treatment rendered accurately and in timely according to K-day PACE policy.
• Measure effectiveness of treatment, document and revise plan accordingly.
• Educate and instruct participants/family in procedures, home programs and document training and understanding by participant/family.
• Confers with physicians and other disciplines to assure a complete and integrated program.
• Completes necessary documentation and statistics accurately and in a timely fashion.
• Direct, supervise, and perform initial and annual competencies, and evaluate the performance of the licensed Physical Therapist Assistant (PTA).
• Review and cosign entries of the PTA into the participant medical record.
• Review the plan of care with the PTA before the PTA provides care. At least once every 30 days after new or revised plans of care, document in the medical record that the plan and the participant’s progress have been reviewed with the PTA.
• Supervise and instruct students and volunteers if assigned.
• Train PACE staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing activities of daily living (ADL’s).
• Perform initial and annual competencies on direct care staff under supervision.
• Participate in and support Quality Initiatives
• Contribute to research and other department activities as assigned.
• Provide age specific competent care to older adults served including knowledge of the aging process.
• Know and adhere to the philosophy and goals of K-day PACE.

• 캘리포니아 주 OTA 자격증 보유자
경력:
• 노인 인구와 최소 1년 이상 함께 일한 경험자 우대

업무 내용:
• 작업치료사(OT) 및 물리치료사(PT)의 지시에 따라 참여자 그룹 활동을 진행
• 부서 유지 관리 및 재고 관리 지원
• 장비 주문 및 발급 보고 (센터 관리자 또는 다학제 팀의 지시에 따라)
• 장비 소규모 수리 수행
• 내구성 의료 장비(DME) 필요성 평가 지원 및 구매 전 팀 승인 획득
• DME 주문 및 후속 조치
• 프로그램 실행 시 다른 직원 및 보호자 교육
• 치료 기술자 및 보조원 지도 및 지원
• 레크리에이션 치료 부서와 활동 적응 관련 협의
• 위원회 참여
• 참여자 정보의 기밀 유지
• 안전 수칙 준수
• 부서 효율성을 위한 관련 업무 수행
• 참여자 요구에 맞춰 일정 조정
• 직원, 참여자, 기관 정보의 기밀 보호
• 품질 개선 노력에 동참
• 기타 지시된 업무 수행
